摘要:
目的:研究基于空间向量的立体定向脑电图(SEEG)电极置入与规划位置偏差的统计和分析方法,以及偏差分布规律。方法:回顾性分析8例进行SEEG电极置入的癫痫患者的临床及电极置入资料,分析SEEG电极CT路径与规划路径的偏差。定义并构建偏差空间向量,应用线性回归方法分析向量模长,向量夹角的偏差,应用向量三视图描述偏差的规律。结果:入点偏差向量模长、靶点偏差向量模长、角度偏差与颅骨的厚度均呈正相关,即颅骨越厚,入点偏差、靶点偏差、角度偏差均越大。向量三视图偏差分析显示,向量分布规律为实际入点沿颅骨形状向后偏,实际靶点位置偏浅。结论:应用空间向量分析方法可很好地分析电极实际置入路径与规划路径的偏差,三视图的方法可以描述空间向量偏差规律,从而能够帮助改进手术规划与操作方案,提高SEEG电极置入的准确性。
Abstract:
Objective:There is always deviation between actual and planned trajectory of stereoelectroencephalography(SEEG) electrodes implantation.To understand the regularity of this kind of deviation,On the basis of the traditional deviation statistics,we propose the deviation statistics and description method based on space vector.To analyze statistics deviations of SEEG electrodes CT trajectories and planned trajectories and to find deviation distribution.Method:The clinical presurgical evaluation and implantation data of 8 patients who were performed SEEG electrodes implantation were analyzed retrospectively.We define and use the deviation vectors to describe the deviation of entry points,target points and angulation between CT trajectory and planned trajectory,and use three views to describe the regularity of deviation.Results:The entry point deviation vectors' length,the target point deviation vectors' length and angulation deviation between CT and planned trajectory are positively related to skull thickness.Three views describe regularity of deviation vectors obviously.The thicker the skull is,the greater the target deviation,CT and planned path angulation deviation will be.The positions of actual target points are shallower than the position of planned target points.The positions of actual entry points are rearward than the position of planned entry points.The entry points deviation change with the shape of skull.Conclusion:Using deviation vectors and three views display deviation vectors can be more complete and accurate describe the regularity of deviation,and thus help to improve the planning and implementation of the electrodes implantation.
