摘要:
目的 评估脑转移瘤患者治疗前常规增强MRI扫描和伽玛刀治疗所采用的薄层增强MRI扫描检测脑转移数量之间的不同。方法 转移瘤的数量在治疗前和治疗中两次扫描图像上分别由2名医生确定,研究分析的两次 MRI图像间隔时间为4周以内。常规的增强MRI扫描为层厚6mm间隔2mm,伽玛刀定位增强MRI扫描是采取对刚性固定框架进行3.0 mm薄层无间隔扫描。结果 1336名研究患者中41.2%的患者在伽玛刀定位时发现转移瘤的数量增加,而数目相同者占58.8%,检测出从单发转移增加到多发转移占13.8%。在结直肠癌、食道癌病例中,单发转移的比例为66.2%,高于其他恶性肿瘤患者。结论 大约有40%的患者在伽玛刀定位薄层图像扫描中被发现转移瘤的数目增加,这对临床上确定脑转移治疗策略有重要意义,因此脑转移瘤患者的薄层MRI增强扫描应当常规使用。
Abstract:
Objective To evaluate differences in the numbers of brain metastases between Thin-slice MRI for gamma knife and conventional contrast-enhanced MRI. Methods The numbers of metastases on the 2 images were counted by two doctors. Images from 1336 patients with an interval between the 2 MRI studies of 4 weeks or less were analyzed. conventional contrast-enhanced MRI is 6-mm slice thickness and Interval of 2-mm , For gamma knife planning, 3mm-slice No interval scanning are taken under rigid frame fixation. Results Increases in the number of metastases were found in 41.2% of the 1336 patients, whereas the number was identical in 58.8%. An increase from single to multiple metastases was found in 13.8%. In cases of colorectal cancer and esophageal cancer, the proportions of patients with a single metastasis 66.2% were higher than that of patients with other malignancies. Conclusions In about 40% of the patients, an increased number of metastases were found on the thin-slice images. Brain metastases from this to determine clinical treatment strategies have important significance. Therefore patients with brain metastases of thin-slice contrast-enhanced MRI scanning should be used as convention.
