摘要:
目的 评价伽玛刀(Gamma Knife)放射外科治疗垂体腺瘤的疗效及安全性。方法 对本中心1999年6月至2002年6月采用OUR旋转式伽玛刀作为首选方法治疗的232例垂体腺瘤患者的临床资料进行回顾性研究,其中包括PRL型89例、GH型53例、ACTH型10例、混合型(GH+PPL)23例和无功能型(NFA)43例。肿瘤直径3mm~67mm(平均21mm),30%~50%等剂量曲线包绕肿瘤,肿瘤平均周边剂量29.6Gy,平均中心剂量41.1Gy。对术后随访患者的临床症状、血液激素水平和鞍区MRI检查结果进行对比分析。结果 176例(75.9%)患者获得随访,随访期3~36个月,平均22月,临床症状改善或消失132例(74.6%),激素水平下降或恢复正常79例(83.2%),肿瘤缩小或消失87例(56.9%),平均肿瘤有效控制率为90.2%。结论 伽玛刀放射外科对各种类型垂体腺瘤的治疗是安全有效的,能够改善和控制大部分患者的临床症状、激素水平和肿瘤生长。
Abstract:
Objective To evaluate the effectiveness and complications of Gamma Knife surgery for treating pituitary adenomas. Methods Retrospective analysis of 232 cases of pituitary adenomas from June 1999 to June 2002 were performed in the Gamma Knife Center of Xi’an Jiaotong University. There were 89 prolactin secreting adenomas(PRL), 53 growth hormone secreting adenomas(GH), 10 adrenocorticotrophic hormone secreting adenomas(ACTH), 23 prolactin/growth secreting hormone (PRL/GH) adenomas and 43 non-functional adenomas. Average maximum tumor diameter was 21mm(3~67mm). The average does in the center of tumor was 41.4Gy and the average does around tumor was 29.6Gy with 30%~50% isodoes line. Results 176 patients were followed up with an average follow-up period of 22 months(3~36 months). Clinical symptoms of 132(74.6%) patients were released or disappeared. Improved endocrine status occurred in 79 cases(83.2%). 87(56.9%) patients experienced arrest of tumor growth. Average control rate was 90.2%. Conclusion Gamma Knife radiosurgery is a safe and effective therapeutic method of achieving clinical symptoms control, endocrine remission/stability and tumor growth control in pituitary adenomas.
