摘要:
目的:应用弥散张量成像(DTI)观察高血压脑出血(HICH)皮质脊髓束(CST)的损伤,探讨DTI在HICH治疗、康复过程中对预后评估的应用价值。方法:对20例HICH患者分别于入院7 d内及康复治疗2个月后,进行DTI检查、CST分级和肌力评级;测定血肿层面CST部分各向异性分数(FA),计算相对FA值(rFA)。结果:HICH患者入院时和发病2个月后rFA值和肌力评级均呈正相关(r=0.703,0.712,均P=0.01);入院时和发病2个月后患侧CST分级与肌力评级均呈正相关(r=0.658,0.649,均P=0.02)。发病后2个月,肌力较好组(肌力4~5级)rFA与肌力较差组(肌力0~3级)比较,差异有统计学意义,所有rFA>0.8的患者运动功能都有显著改善。结论:通过DTI的FA测定,可以了解HICH患者CST的损伤情况,有助于早期评估HICH患者的肌力恢复情况和预后。
Abstract:
Objective:We prospectively investigated the predictive value of diffusion tensor imaging (DTI) for motor functional outcome in a case series of patients with hypertensive intracerebral hemorrhage (HICH).Methods:DTI was performed in 20 patients with intracerebral hemorrhage within one week after onset.Mean fractional anisotropy (FA) values along the corticospinal tracts(CST) at the level of the hematoma were measured bilaterally,and the ratios of values (affected side / healthy side) were determined as relative FA value (rFA),Patients were evaluated for motor function on admission and at 2 months after onset using the manual muscle test (MMT) score and then divided into good (MMT,4-5) and poor (MMT,0-3) motor function groups.Results:The rFA value,CST grade,and limb muscle strength were positively related with the injured CST side at admission and the onset of 2 months.(r=0.703,0.712,P=0.01;r=0.658,0.649,P=0.02)The rFA value in the group with good motor functional outcome were significantly higher than those in the group with poor motor functional outcome (P<0.01) .All patients who showed higher than 0.8 rFA value had a good motor functional outcome.Conclusion:Motor functional outcome in patients with intracerebral hemorrhage can be predicted by measuring FA values using diffusion tensor imaging.
