【摘要】目的探讨急性乙醇中毒对创伤性脑损伤(TBI)患者血清水通道蛋白-4(aquaporin-4,AQP4)及胶质纤维酸性蛋白(glial fibrillary acidic protein,GFAP)表达水平的影响,及其临床意义。方法选取中重型创伤性脑损伤患者36例,其中合并急性乙醇中毒患者18例(乙醇中毒组),未合并急性乙醇中毒患者18例(无乙醇中毒组);以及健康体检者16例(正常对照组)。应用ELISA法检测患者伤后12 h内及伤后24 h、48 h血清AQP4、GFAP水平,分析其变化趋势及与病情发展和预后的关系。结果乙醇中毒组患者各时间点血清AQP4、GFAP水平均高于无乙醇中毒和正常对照组;且乙醇中毒组患者伤后6个月的GOS评分较无乙醇中毒组患者明显降低(预后更差);差异均有统计学意义(均P<0.05)。乙醇中毒组患者中,血乙醇浓度越高者的血清AQP4、GFAP水平也越高(均P<0.05)。结论急性乙醇中毒可引起创伤性脑损伤患者的血清AQP4、GFAP水平明显增高,而使患者病情加重,预后变差。并且患者血乙醇浓度越高,其血清AQP4、GFAP水平也越高,伤情越重,预后越差。监测急性乙醇中毒合并创伤性脑损伤患者的血清AQP4、GFAP水平改变,可为伤情准确评估和有效治疗提供依据。
Abstract:
Abstract: ObjectiveTo investigate the effect of acute ethanol intoxication(AEI) on the expression of AQP4 and GFAP in serum after traumatic brain injury(TBI) in patients. The clinical significance was also investigated. Method36 patients were selected for TBI, of which 18 were combined with AEI(observation group), 18 were not combined with AEI(control group 1) and 16 were examined(control group 2). ELISA was used to determine the AQP4 and GFAP levels in the serum of 12 h and 24 h and 48 h after injury. ResultsThe levels of AQP4 and GFAP in the observation group were higher than those in the control group at each time point, and the GOS score in the observation group was significantly lower than that in the control group(the prognosis was worse) (all P<0.05). The higher the concentration of ethanol was in the observation group, the higher the expression levels of AQP4 and GFAP were in the serum(all P<0.05). ConclusionAEI can increase the expression of AQP4 and GFAP in serum of TBI patients, aggravate their condition and worsen their prognosis. The higher the concentration of ethanol is in blood, the higher the expression of AQP4 and GFAP is in serum, the more severe the injury is and the worse the prognosis is. It provides a basis for accurate assessment and further comprehensive treatment of patients with AEI combined with TBI.
