摘要:
目的探讨MRI纹理分析对术前鉴别脑膜瘤分级的价值,为指导进一步治疗提供有效依据。方法回顾性分析徐州医科大学附属医院2013年1月—2018年5月收治的59例脑膜瘤患者的临床资料。患者均术前行MRI检查,经手术切除肿瘤,术后病理检查证实并明确分型。在MRI图像的肿瘤最大层面上,用Firevoxel软件勾画出肿瘤的实体部分,并测量其平均值、标准差、偏度、峰值、不均一度和熵值。采用独立样本t检验、Mann-Whitney U检验对低级别与高级别脑膜瘤进行检验,并对纹理参数进行ROC分析,评估检验效能。结果直方图参数中,低级别脑膜瘤的均数、标准差、偏度值、峰度值分别为2 753±1 018.50、450.00(348.25,563.57)、-0.68(-1.38,-0.34)、1.36(0.49,3.99);高级别脑膜瘤分别为1 994.02±982.44、353.75(266.18,463.89)、-0.39(-1.09,-0.18)、0.99(-0.37,2.18);两组均数间比较,差异有统计学意义(t=2.831,P=0.006)。纹理参数中,低级别脑膜瘤的不均一度、熵值为0.18±0.04、3.59±0.44;高级别脑膜瘤为0.23±0.06、4.08±0.24;两组之间比较,差异均有统计学意义(t=4.056,P=0.000;t=4.841,P=0.000);对应曲线下面积分别为0.765、0.821。结论纹理分析可在术前较为准确地鉴别出低级别与高级别脑膜瘤。
Abstract:
Objective Preoperative prediction of grade of meningioma is of great significance for treatment planning. The purpose of this study was to evaluate preoperatively the grade of meningioma by texture analysis and to provide a solid basis for further treatment. Methods A retrospective study was performed on 59 patients with meningioma who underwent preoperative MRI examination, surgical resection and postoperative pathologic confirmation in the Affiliated Hospital of Xuzhou Medical University from January 2013 to May 2018. At the maximum level of MRI images, Firevoxel software was used to delineate solid parts of the tumor and to measure Mean, SD, Inhomogeneity, Skewness, Kurtosis and Entropy on enhanced T1WI sequences. Independent sample t test and mann-whitney U test were used to examine low-grade and high-grade meningiomas. ROC statistical analysis was conducted for texture parameters to evaluate the test efficacy. Results Among the histogram parameters, the mean value, standard deviation, skewness value and kurtosis value of low-grade meningioma in the enhanced T1WI sequence were 2753 ±1018.50, 450.00(348.25, 563.57), -0.68(-1.38, -0.34), 1.36(0.49,3.99)respectively. High grade meningiomas were 1 994.02 ± 982.44, 353.75(266.18, 463.89), -0.39(-1.09, -0.18), 0.99(-0.37,2.18)respectively. The difference between the two groups was statistically significant (t=2.831, P=0.006). In the texture parameters, the heterogeneity of the enhanced T1WI sequence, the entropy of the low-grade meningioma was 0.18 ± 0.04 and 3.59±0.44. High grade meningiomas were 0.23±0.06 and 4.08±0.24. The two parameters had statistical significance between the two groups (t value was 4.056 and 4.841, P value was 0.000 and 0.000, respectively). The AUC is 0.765 and 0.821 respectively. Conclusion Texture analysis can accurately determine the accuracy of different grade of meningioma before surgery.
