摘要:
目的探讨额叶胶质瘤切除术后瘤腔积液导致急性高颅压症的原因与处理。方法回顾分析1例大脑胶质瘤病行额叶病灶切除术后出现张力性瘤腔积液患者的临床资料;并对相关文献进行复习。结果本例患者瘤腔积液形成的原因为手术造成自额角指向瘤腔的单向“活瓣”结构,形成张力性瘤腔积液,压迫邻近脑组织。通过瘤腔外引流封闭额角,最终囊腔内液体参与脑脊液循环,使得张力性积液得到根本的解决,避免了再次开颅手术。结论瘤腔置管外引流术是治疗脑肿瘤术后急性张力性瘤腔积液经济、有效的手术方式。
Abstract:
Objective To explore the cause and treatment of acute intracranial hypertension induced by tension effusion in tumor cavity after frontal glioma-resection. Method The clinical data of one patient of tension effusion in tumor cavity after frontal glioma-resection was analyzed retrospectively. The relevant literatures were reviewed. Results In this case,cerebrospinal fluid was retained in the tumor cavity by a structure of one-way “valve” from the frontal horn to the tumor cavity due to surgery, which then forming a tension effusion in tumor cavity and compressing adjacent brain tissue. The fluid was drained outside from the tumor cavity, and finally the liquid in the cystic cavity participated in the cerebrospinal fluid circulation. This made the tension effusion get a fundamental solution, avoiding the second craniotomy. Conclusion External drainage of the tumor cavity is an economical, effective treatment for acute tension effusion in tumor cavity after brain tumor resection.
