摘要:
目的探讨血管内皮生长因子(VEGF)、金属基质蛋白酶-9(MMP-9)、上皮间质化(EMT)相关蛋白在脊柱转移瘤的表达及其相关性。方法采用免疫组化检测90例脊柱转移瘤患者的肿瘤组织和瘤旁正常组织的VEGF、MMP-9、EMT相关蛋白(E-Cadherin,N-Cadherin)的表达水平。将患者按肿瘤组织的来源分为上皮来源组(27例)和间叶组织来源组(63例),按Tomita评分分为预后良好组(Tomita评分≤5,54例)和预后不良组(Tomita评分>5,36例)。比较各组患者的VEGF、MMP-9、EMT相关蛋白表达阳性率,分析其表达阳性的相关性。结果肿瘤组织的VEGF、MMP-9、N-Cadherin表达阳性率(75.6%、88.9%、70.0%)显著高于正常组织(52.2%、18.9%、14.4%),E-Cadherin表达阳性率(55.6%)明显低于正常组织(95.6%),差异均有统计学意义(均P<0.001)。上皮来源组患者的VEGF、MMP-9、N-Cadherin表达阳性率明显高于,E-Cadherin表达阳性率明显低于间叶来源组患者,差异均有统计学意义(P<0.05~0.001)。预后不良组患者的VEGF、MMP-9、N-Cadherin表达阳性率明显高于,E-Cadherin表达阳性率明显低于预后良好组,差异均有统计学意义(P<0.05~0.001)。Spearman相关分析显示,VEGF与MMP-9、N-Cadherin表达阳性呈正相关(r=0.210,P<0.05;r=0.17,P=0.001),与E-Cadherin表达阳性呈负相关(r=-0.353,P<0.001);MMP-9与N-Cadherin表达阳性呈正相关(r=0.231,P<0.05),与E-Cadherin表达阳性呈负相关(r=-0.245,P<0.05)。结论VEGF、MMP-9、EMT相关蛋白在脊柱转移瘤的表达异常,并与肿瘤转移过程及预后密切相关,可作为临床评价肿瘤状况的重要参考指标。
Abstract:
To investigate the expression and correlation of vascular endothelial growth factor(VEGF), matrix metalloprotein-9(MMP-9) and epithelial mesenchymal transformation(EMT)-related proteins in metastatic spine tumors(MST). Methods Immunohistochemistry was used to detect the expression of VEGF, MMP-9 and EMT-related proteins(E-Cadherin, N-Cadherin) in different tissues. According to the Tomita score, patients were divided into two categories: good prognosis(Tomita score≤5, n=54) and poor prognosis(Tomita score>5, n=36). The correlation between their expression in spinal metastases and the prognosis was analyzed. Results The positive rates of VEGF, MMP-9 and N-Cadherin expression in tumor tissues were 75.6%, 88.9% and 70.0%, respectively, which were higher than 52.2%, 18.9% and 14.4% in normal tissues. The positive rate of E-Cadherin in tumor tissues was lower than that in normal tissues(55.6% vs. 95.6%, P<0.001). The positive rates of VEGF, MMP-9, and N-Cadherin expression in patients with epithelial origin were higher than those in patients with mesenchymal origin in spinal metastases, and the expression of E-Cadherin was lower in patients with mesenchymal origin(P<0.05-0.001). The positive rate of VEGF, MMP-9, N-Cadherin expression in patients with poor prognosis was higher than that in patients with good prognosis(P<0.05-0.001). Spearman correlation analysis showed that VEGF was positively correlated with MMP-9 and N-Cadherin positive expression(r=0.210, P<0.05, r=0.17, P=0.001), and negatively correlated with E-Cadherin positive expression(r=-0.353, P<0.05). There was a positive correlation between MMP-9 and N-Cadherin expression(r=0.231, P<0.05), and negative correlation with E-Cadherin positive expression(r=-0.245, P<0.05). Conclusions VEGF, MMP-9 and EMT-related proteins are abnormally expressed in MST, which is closely related to tumor metastasis and can be used as an important reference for clinical evaluation of tumor status.
