摘要:
目的探讨高压氧(HBO)治疗小儿亚急性硬膜下血肿脑复张的疗效。方法将2016年1月—2018年1月收治的50例亚急性硬膜下血肿患儿分为高压氧治疗组和对照组,每组各25例;两组患儿的病情程度无明显差异。治疗组患儿在常规药物治疗的基础上行高压氧治疗,对照组患儿采用常规神经营养药物治疗。根据治疗后头颅CT或MRI测量的硬膜下残腔最大层面厚度(T),将脑复张的程度分为3级(Ⅰ级:T<0.5 cm,Ⅱ级:0.5 cm≤T<1 cm,Ⅲ级:T≥1 cm)。分别比较两组患儿治疗后1、3、6个月时脑复张程度的差异。结果治疗后1个月时两组患儿脑复张程度的差异无统计学意义(P>0.05);治疗后3个月、6个月时两组脑复张程度的差异均有统计学意义(均P<0.05)。结论高压氧治疗能有效促进亚急性硬膜下血肿的脑复张;应以高压氧联合神经营养药物作为其常规治疗的措施。
Abstract:
To investigate the hyperbaric oxygen(HBO) therapy on brain reexpansion of subacute subdural hematoma in children. Methods 50 children with subacute subdural hematoma admitted from January 2016 to January 2018 were randomly divided into HBO treatment group and control group with 25 cases each. There was no significant difference in the sevesity of illness between the two groups. Treatment group were treated with medication and HBO therapy. Control group were treated with regular drug treatment. According to the maximum thickness(T) of subdural residual space measured by CT or MRI, the degree of brain reexpansion was divided into three grades(grade Ⅰ: T<0.5 cm, grade Ⅱ: 0.5 cm≤T<1 cm, grade Ⅲ: T≥1 cm). The degree of brain reexpansion was compared between the two groups at 1, 3 and 6 months after treatment. Results There was no significant differences on the brain reexpansion degree between two groups after 1 month(P>0.05). The results were significant difference in the brain reexpansion degree after three and six months(P<0.05). Conclusion Hyperbaric oxygen can effectively promote the brain reexpansion combined with neurotrophic medication, the combined therapy can be used as routine treatment method.
