摘要:
目的探讨不同时机高压氧联合亚低温治疗重型颅脑损伤(sTBI)的疗效及对相关血清指标的影响。方法沧州市中心医院2018年2月—2019年3月收治的82例sTBI患者,根据治疗方式分为研究组及对照组,每组41例。研究组患者采用早期高压氧联合早期亚低温治疗;对照组患者采用早期亚低温联合后期高压氧治疗。比较两组患者的临床疗效、预后,以及相关血清指标的差异。结果治疗后3个月,研究组患者的总有效率(90.2%)显著高于对照组(73.2%)(P<0.05)。治疗前两组患者的组织氧分压及颅内压的差异无统计学意义(均P>0.05),治疗后3个月研究组患者的组织氧分压明显高于、颅内压明显低于对照组(均P<0.05)。治疗后3 d研究组患者的相关血清指标均较对照组明显改善(均P<0.05)。治疗后3个月时,研究组患者的预后显著优于对照组(P<0.05)。结论早期高压氧联合亚低温治疗sTBI的疗效更为显著,不仅可有效降低颅内压,改善脑组织缺氧状况,还可以明显改善患者的预后,值得在临床推广应用。
Abstract:
To investigate the effect of hyperbaric oxygen combined with mild hypothermia in the treatment of severe traumatic brain injury(sTBI) and its influence on the related serum indexes. Methods From February 2018 to March 2019, 82 patients with sTBI admitted in Cangzhou Central Hospital were divided into study group and control group with 41 cases in each. Patients in control group were treated with sub-hypothermia in the early period combined with hyperbaric oxygen therapy in the late period. Patients in study group were treated with sub-hypothermia combined with hyperbaric oxygen therapy in the early period. The clinical efficacy, prognosis and related serum indexes were compared between the two groups. Results Three months after treatment, the total effective rate of the study group(90.2%) was significantly higher than that of the control group(73.2%) (P<0.05). There was no significant difference in tissue oxygen partial pressure and intracranial pressure between the two groups before treatment(all P>0.05). Three months after treatment, the tissue oxygen partial pressure of the study group was significantly higher than that of the control group, and the intracranial pressure was significantly lower than that of the control group(all P<0.05). Three days after treatment, the related serum indexes of the study group were significantly improved compared with the control group(all P<0.05). Three months after treatment, the prognosis of the study group was significantly better than that of the control group(P<0.05). Conclusions Early hyperbaric oxygen combined with mild hypothermia is more effective in the treatment of STBI. It can not only effectively reduce intracranial pressure, improve the hypoxia of brain tissue, but also significantly improve the prognosis of patients, which is worthy of clinical application.
