癫痫患者常常在发作时出现意识障碍。这可能涉及大脑响应性的改变,或自我意识和主观体验的改变。上行网状激活系统和近边缘额顶联合皮层被认为是产生意识的解剖基础。网络抑制假说提出了在失神发作、复杂部分性和强直-阵挛性癫痫发作期间,意识受损的共同神经解剖学基础。迷走神经电刺激(VNS)、脑深部电刺激(DBS)、颅内皮层刺激(DCS)和重复经颅磁刺激(rTMS)是目前公认的治疗癫痫的神经调控手段。在临床研究中,VNS植入受试者,对于警觉性增加和记忆巩固的疗效都已有报道;单侧苍白球DBS在麻醉的个体中可引起短暂的清醒;但仍需要研究刺激参数变化对意识和认知的影响,以及如何发挥疗效改善患者的生活质量。
Abstract:
The consciousness of epileptic patients is often disturbed during the onset. This may involve in brain responsiveness or changes in self-awareness and subjective experience. The ascending reticular activation system and the near-marginal frontal-parietal syncortex are considered to be the anatomical basis of consciousness. The network inhibition hypothesis provides a common neuroanatomical basis for impaired consciousness during absence seizure, complex partial seizures and generalized tonic-clonic seizures. Vagus nerve stimulation(VNS), deep brain stimulation(DBS), intracranial cortical stimulation(DCS) and repetitive transcranial magnetic stimulation(rTMS) are currently recognized the effect treatment of epilepsy. In clinical studies, the short-term and long-term effects of VNS implantation on increased consciousness have been reported. The choice of bilateral frontal lobes or central thalamic nuclei by DBS does not seem to affect cognition. Unilateral globus pallidus DBS can cause temporary awakening in anesthetized individuals. With the development of implantable neuromodulation technology, thalamic DBS is increasingly established as a clinical intervention. However, it is necessary to study the effects of changes in stimulation parameters on consciousness and cognition, and how to improve patients’ quality of life.
